The Long Term Care “Tool” is a 7 page health scoring form which, when broken down, has 5 ‘practical’ sections.

  1. Screening
    (desk check flowchart to find out if worthwhile evaluating)
  2. Evaluation for eligibility
    (Score for: Adherence, Health Conditions, High Risk Meds, Hospital Admissions. Eligible patient score >= 20)
  3. Medicines Management Plan
    (Patient’s capability and situation. )
  4. Agreed Actions
    (Patient agreement to: synchronisation, dispensing frequency, med reconciliation, chart, reminders, packs, next review)
  5. Registration
    (Patient sign-off must be signed before setting LTC Start date)


1. Screening

Toniq Template: LTC1 Screening

Pharmacies familiar with the LTC process do not use this part of the tool very much. It helps to identify that a patient is NOT eligible without going through detailed scoring. (Scroll down when searching for this template as it is not grouped with the other commonly used LTC templates.)


2. Evaluation

Scoring for LTC eligibility. Scores are made for broadly 4 sections: Adherence(including patient factors), Health conditions, High risk medicines and Hospital admissions. Scoring must result in at least 20 points for automatic eligibility. Adherence and Conditions should be loaded onto patients before starting evaluations. Eligible patients must have a medicine plan done and agree management points with the patient or the patient’s agent.


3. Medicine Management Plan

Review current medicine management and identify issues facing patients who are eligible to receive the service.


4. Action Plan

Action Points Review current medicine management and identify issues facing patients who are eligible to receive the service. The Patient will be expected to sign off on these. Relevant sections of the tool will need to be sent to the patient’s GP.


5. Registration

Load template, fill in and print for patient to sign. Set LTC Start date on patient record. 



NB: LTC factors and conditions should be saved onto your patient BEFORE the evaluation is started so that 90% of this template autopopulates and autoscores.
